Homeward Bound offers a menu of aftercare solutions that are affordably priced. Each level of care is customized in its intensity to meet your family’s unique needs. The length, cost and basic overview of each program is provided. In addition, the “Family Fit” column is intended to provide you with an idea for which program might be right for your family.
We encourage you to allow us to work with you, your educational consultant, and your teen’s program therapist to help you identify the program that is right for you.
To ensure availability and to help you get matched with a specialist that is best for your teen and family, we suggest you arrange for our services at well in advance of your teen’s discharge/graduation. This also allows your Homeward Bound specialist to begin working with your consultant, program therapist and your family to effectively lay the foundation for a successful transition home.

Transition Essentials™
Families receive scheduled coaching to help them navigate the unforeseen pitfalls of transition. They are given access to the Family Bridge™, an innovative, online environment that facilitates distance coaching, case coordination, learning and communication.

Direct Access™
Prior to a teen's discharge, our experts work with parents and therapists to design a successful transition plan. Families receive scheduled coaching as they execute the plan. The most unique element of this package is the "in-the- moment" access to Homeward Bound Transition Specialists during the times of crisis.

In-Home Support
Often times a teen's transition home is met with complicated family dynamics that can interfere with successful execution of the transition plan. In- Home Support offers families our most intensive, hands-on solution. In addition to Scheduled Coaching, Family Bridge and Direct Access, our experts conduct an in-home workshop teaching families the essentials of success. These include: communication, rules/consequences, and organizing natural mentors and professional support. We augment our standard service with customized practices to address immediate needs - unifying the family and keeping them moving in the right direction.
