Fundamental to Family Transition Management™ is the understanding that any system without proper maintenance will began to fall apart. A family system is no different, so at the core of FTM™ is an effort to instill in the family unit a process for maintenance. Families learn to make ongoing improvements for the rest of their lives. Using the model of FTM™, Homeward Bound Specialists restart family units and help them develop a process, proven in structure but unique to their needs, that allows them to deal more effectively with their problems. There are no magic tricks. It is simply a matter of taking evidence and experience and putting it to work.
Whatever elements a family includes in their specific process, it must follow the scientific method, or a continual improvement process. Time-tested for centuries, this is the best way to achieve one’s desired results.

After the Homeward Bound Specialist makes a complete and thorough assessment of the child or teen, the family system, and the child or teen’s ecosystem, the Specialist works with the family to discuss the evaluation and form a behavioral regimen at their family planning meeting.
The behavioral regimen works out the daily activities of the child or teen, as well as portions of the parents’ and other family members’ daily activities. These activities include Homeward Bound training, education, coaching, and family exercises, but for the child or teen, the regimen also outlines his or her day from morning to night.
Throughout the week positive individuals including peers, teachers, extended family, and others (The Home Team) casually monitor the child or adolescent’s behavior to assist with the continual assessment.
After the evaluation, a family planning meeting is held to review and make changes to the behavioral regimen. This effort continues for the duration of the program.
